Our office is closed for lunch daily from 12:00pm until 1:00pm
You can reach our office by telephone at (540) 423-6600 between the hours of 8:30am and 5:00pm. After 5:00pm and after hours, you can reach our answering service by dialing our main number.
All new patients must arrive at least 15 minutes early for registration. You may download and complete our registration forms in advance and bring them with you to your appointment.
If you arrive more than 15 minutes late for your appointment, you may be asked to reschedule. This helps to ensure that each scheduled patient can be seen in a timely fashion. We request 24 hour notice on all appointment cancellations.
We prefer that you refill your medications at the time of your office visit. Please bring a medication list or your medications with you to each appointment.
Due to a large number of refill requests we receive, it may take us up to 72 hours(3 business days) to call or fax in your prescriptions. Please be sure to give us at least 72 hours notice on all refill requests.
If you have not seen the physician in 6 months and are completely out of your medications, an office visit is required before your prescriptions are refilled.
Have your mail order company fax your refill request to the doctor and we will make every effort to sign it and refax for you within the 72 hour refill time. Remember, we do not call mail order pharmacies.
Medical records are confidential, therefore, we will not release any of your medical information without first obtaining a signed authorization from you. You may come into the office at any time to obtain and complete your medical records request/authorization.
In addition, there is a fee of $.50 per page for the first 50 pages and then $.25 per page thereafter.
